About:
At Creek Valley Health Clinic, our mission is to promote healthier lives through affordable, accessible integrated healthcare.

Creek Valley Health Clinic has a goal to make our community a better place through providing affordable healthcare services, education, and other resources at an affordable rate to community members.

We are a 501(c)(3) nonprofit medical corporation managed by a Board of Directors who all live and work in ‘Creek Valley’ (Apple Valley, Hildale, Colorado City, Centennial Park, and Cane Beds). In order to best serve the community, the organization has submitted many various grants, and have recently received a Program 330 grant award to become a Community Health Center (FQHC). As a primary care clinic, we will offer annual checkups, well child services, sports physicals, vaccinations, and other preventive medical services. In addition to primary care, we plan to offer preventive dental care, behavioral health services, improved pharmacy access, and specialty services such as: imaging, laboratory, cancer screenings, case management, transportation, health education, and insurance eligibility assistance.

As a nonprofit community health center:

Creek Valley Health Clinic will not deny services based on a patient’s inability to pay. Creek Valley Health Clinic will not deny services for any reason – including: race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity. Creek Valley Health Clinic offers discounts based on family size and income. We call this our ‘HealthShare Program’ - which is available to all patients, regardless of income or insurance status.
